Multimodal analgesia is based on the premise that the concurrent use of primarily nonopioid analgesics can have additive, if not synergistic, effects that produce superior analgesia while decreasing opioid use and opioid-related side effects. Multimodal regimens involve the use of multiple medications (e.g., opioids and nonopioids) with different mechanisms of action ( Figure 2) as well as nonpharmacologic interventions to achieve more effective analgesia. Use of multiple analgesics allows for lower and safer doses of each drug ( Jarzyna et al., 2011 ). Multimodal analgesia is a term that refers to the use of several different analgesic medications or techniques simultaneously, to target multiple receptors within nociceptive and neuropathic pathways, thus reducing acute postoperative pain and the surgical stress response, as well as potentially impacting the mechanistic chain of events that can lead to chronic postoperative pain . The effort to provide opioid‐reduced, or opioid‐free, anaesthesia by the application of multimodal Under this approach, called “multimodal general anesthesia,” the additional drugs may include agents with specific central nervous system targets such as dexmedetomidine 8 as well as ones whose targets are less specific such as lidocaine.

Multimodal techniques for pain management include the administration of two or more drugs that act by different mechanisms for providing analgesia, and regional analgesia techniques. The drugs may be administered via the same route or by different routes, and the choice of medication, dose, route, and duration of therapy should be individualized.

Furthermore, poor acute pain management is a strong risk factor for the development of chronic pain. A multimodal approach is recommended, using multiple strategies before, during and after surgery.

The drugs may be administered via the same route or by different routes, and the choice of medication, dose, route, and duration of therapy should be individualized. Multimodal analgesia is a component of many care pathways or Enhanced Recovery After Surgery protocols. The assumption is that combining various nonopioid analgesics will reduce the need for opioids and consequently opioid-related complications.
